Dakota Communities Access Program (DCAP) Initiative
DCAP was a three-year, $2.6 million federally-funded DMF initiative to improve access to and coordination of healthcare services for uninsured and underinsured persons in central and eastern North Dakota. From 2003-2007, DCAP focused on four primary goals: 1) public health insurance enrollment, 2) prescription assistance, 3) dental care access and 4) diabetes quality improvement.
To achieve these goals, DCAP placed community resource coordinators (CRCs) in hospitals, clinics, Federally Qualified Healthcare Centers, and public health departments. CRC’s work one-on-one with people to help them access critical health and financial programs.

Results:
- 13 of 15 sites are sustaining the CRC positions
- 6,107 people received health coverage application assistance
- 3,140 people were enrolled in low-income coverage programs
- 7,590 people received prescription application assistance
- 8,661 people received application assistance for other programs
- 2,515 health education events were held
- 458,263 people were educated through outreach efforts
- $11,827,290 reimbursed to North Dakota healthcare facilities as a result of health
coverage enrollments
- $7,883,935 worth of medications accessed through prescription assistance
- $143,138 in free diabetes testing supplies distributed to low-income persons
